Havenwood Care of Michigan LLC ("we," "us," or "our") is a private home care agency located at 120 North Washington Square, Suite 300, Lansing, MI 48933. This policy explains what information we collect through havenwoodmichigan.com, how we use it, and the choices you have. We keep it in plain English on purpose.
Information we collect
When you fill out the form on our website, you may give us:
- Your name, phone number, and email address
- Who needs care and your relationship to them
- Whether that person has Michigan Medicaid and whether they have Medicare
- The county where they live
- What daily activities they need help with
- Whether you live in the same home, and who would be the caregiver
- Anything else you choose to write in the message box
Some of this is information about a person's health and benefits. We only receive what you type. You never have to share medical details through the website to talk with us. You can call us instead.
We also record how you found us (for example, a flyer or a web link) so we know which outreach is working. Our web host may keep basic technical logs such as IP address and browser type.
How we use it
- To call you back and answer your questions
- To check whether the Michigan Home Help program, or another program, may fit your family
- To help with enrollment and paperwork if you decide to move forward
- To meet our legal, employment, and Medicaid provider obligations
What we never do
- We never sell your information.
- We never share it with anyone for their marketing.
- We do not place advertising or tracking pixels on the page where you fill out the form.
How form submissions are delivered
When you press Send, your answers go through Web3Forms, a form delivery service, to our business email inbox. These providers handle the data only to deliver it to us.
When we share information
If you choose to move forward, we share what is needed with the Michigan Department of Health and Human Services and other parties required to enroll you, run background checks, and process payroll. We may also share information when the law requires it.
Keeping it safe and how long we keep it
Our website uses HTTPS, and only the people who need your information to help you can see it. If you contact us but do not move forward, you can ask us to delete your information at any time. If you do move forward, we keep records as long as Michigan Medicaid and employment rules require.

Children
This website is meant for adults. We do not knowingly collect information from children under 13.
Changes
